Qualifying conditions under the Utah Medical Cannabis Act:
** Note: Patients with medical conditions not listed in the law may petition the compassionate use Board for a medical cannabis card, which will review complete petitions and recommend eligibility on a case-by-case basis. LMPs are unable to submit to the Compassionate Use Board. **
You must register for a medical cannabis card with the Department of Health's Electronic Verification System. You may find this difficult to navigate, we are happy to help you through this process and answer any questions you may have during this step of the process.
Schedule an initial consultation with our medical provider to discuss your eligibility for a medical cannabis card and receive your same-day certification.
* Must be 21 years old with a qualifying medical condition.
** Contact our office for additional information on petitions for the compassionate use board.
If approved, your certification will be completed online with the Department of Health so you can obtain your card the same day.
The initial certification of medical cannabis is valid for 6 months. Once you've held your medical cannabis card for 1-year, follow-up visits may be annual.
